WHAT IS IT?
It is a multidisciplinary diagnostic and treatment process that evaluates medical, psychological, social, cognitive and functional capabilities of an elderly person to optimise care to achieve improvement in health with ageing.
It differs from a standard medical evaluation in the following ways:
- It is focused on an elderly person with complex and multiple medical problems
- It is person centred approach and care
- It emphasises functional status and quality of life
- It takes considerable time and expertise and focuses on challenges like intellectual impairment, mobility, incontinence, iatrogenic disorders and interactions with medications.
