I have developed a simplified model of care to identify people who would benefit from a Comprehensive Geriatric Assessment.

The presence of any of the following conditions in an elderly person may warrant an assessment.

C – Continence problems
O – Orientation-Cognition/Memory difficulties
F – Falls – more than 1 per year
F – Five or more medications
S – Sad mood, Sensory impairment – Vision & hearing difficulties

What is a Comprehensive Geriatric Assessment?

It is a multidisciplinary diagnostic and treatment process that evaluates medical psychological, social, cognitive and functional capabilities of an elderly person to optimise care and to achieve improvement in health with ageing.

  • A valid GP referral is required to attend a specialist appointment and to receive a Medicare rebate, please request a referral from your GP to be sent to Coffs Aged Care
